The RoleUnder the general direction of the Program Director or Designee, the Case Manager is responsible for aiding clients requiring care for substance use disorder. In collaboration with clients, interview and evaluate clients identifying best patient care practices. This work is conducted in support of the mission and goals of Samaritan Daytop Village. ResponsibilitiesWhat You Will DoReviews all documentation establishing the client's eligibility for the program. Facilitate admissions into the Opioid Treatment Program Creates and maintains client files. Conducts initial intake and psycho-social assessment of clients' needs and periodic re-assessments. In collaboration with clients, prepares initial and periodic revisions of short-term and long-term client goals. Assists clients in attaining their goals by referring them to identifying and locating community resources for clients, and making referrals to appropriate services Works with clients to break through barriers to client goals, and assists clients in advocating for themselves and in moving toward self-sufficiency. Monitors clients' progress toward their goals (dates achieved) via regularly scheduled contact and document via progress notes. Participate in both individual and group counseling sessions Assist in formulating regular discharge planning and referrals in support of continued care Collection of toxicology samples Participation in regular team meetings, case conferences and other training Follows up with clients and with referral organizations regarding client contact and progress with referral organization. Provides all required information for weekly/monthly/quarterly/annual reports. Acts as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, childcare, housing, legal issues, etc. Additional responsibilities as assigned QualificationsWho You Will BeSomeone with a High School Diploma or equivalency and 2+ years of case management experience. OR Associate Degree with One-Two (1-2) years of case management experience OR Bachelor's Degree with at least One (1) year of case management experience. CASAC-T, CASAC-P or CASAC Ability to consistently maintain required documentation. Ability to maintain confidentiality. Working knowledge of NYC's diverse network of community resources. Must demonstrate sensitivity to the needs of clients in crisis. Experience collaborating directly with people from diverse racial, ethnic, and socioeconomic backgrounds. li-onsite
