Clinical background in acute care is helpful but not required. Candidates must have experience with Epic. Educating patients and caregivers to facilitate discharge is essential. The role involves coordinating services to home care and equipment following discharge. It includes arranging transfers to physician appointments. The average case load is 18-24 patients, with weekends potentially increasing to 30 patients. This role will support the clerical needs of the Case Management Department. It focuses on insurance authorizations and post-acute care referrals. Working hours may flex slightly during the shift. No weekend work is required for this position. Travelers are expected to float between various like units within their skill set. Reference requirements include two references covering one year within the last three years.
