If you thrive in a collaborative environment and are driven to improve quality of life for others, this is the opportunity for you.Position SummaryThe Home Care Coordinator (LPN) plays a vital role in coordinating, implementing, and monitoring home-based services for participants. This position ensures delivery of high-quality, person-centered care aligned with individualized care plans and supports participants across all care settings.You'll serve as a key liaison between participants, caregivers, and the interdisciplinary team to ensure seamless, compassionate care every step of the way.Key ResponsibilitiesCoordinate and oversee all home care services in alignment with participant care plans.Conduct in-home visits and assist with participant assessments and reassessments.Collaborate with the interdisciplinary team (IDT) to support clinical, functional, and psychosocial needs.
