Role Summary The Primary Care Physician (PCP) is accountable for outcomes, clinical care, and cost-effectiveness for patients. The PCP builds trust, helps patients change behaviors, sets health goals, and leads their care team to achieve measurable results in patient outcomes, experience, and cost management. Essential Duties and Responsibilities Provide care for patients with acute and chronic illnesses in the older adult population. Take full accountability for patient care and outcomes, consulting with specialists as needed. Engage with hospitalists for hospitalized patients. Lead a care team including care promoters, facilitators, and coordinators. Collaborate with transitional care teams, case managers, and other healthcare professionals for patients unable to visit the office. Participate in managing the medical center and covering for other providers as needed. Support recruiting efforts for patients and providers. Supervise and collaborate with Advanced Practice Practitioners (APPs). Participate in meetings, quality programs, and compliance initiatives. Competencies Availability and accessibility to patients. Fluency in English (additional language requirements may apply). Education and Experience MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty. Active or eligible state medical license; DEA certification required. Board certification preferred; board eligibility required. Maintain certification through MOC/CME requirements. Basic Life Support (BLS) certification required within 90 days of employment.
