Hourly Pay: $37.50 - $41.50 / per hourLicensed Practical Nurse (LPN) - Transition TeamEmployment Type: Full-Time & Part-Time AvailableMaxim Healthcare is proud to be recognized as a Certified Great Place to Work - 2026, and we are hiring Licensed Practical Nurses (LPNs) to join our specialized Transition Team.About the Transition TeamOur Transition Team is a supplemental care unit of highly skilled nurses whose primary focus is to quickly and efficiently support patients transitioning home while permanent nursing staff are actively recruited. This team is deployed immediately after a patient referral is received and the patient is admitted to Maxim.Transition Team nurses provide nursing care for all authorized hours in the home during a provisional period of 1-90 days. During this time, our office team recruits permanent caregivers. Once identified, those caregivers are given the opportunity to train and shadow alongside Transition Team nurses in the home before assuming independent care.
