Occupational Therapist (Inland Empire) Home Health

We are seeking a compassionate and skilled Occupational Therapist to join our home health team on a per diem basis. This role involves providing rehabilitative services to patients in their homes to help them regain independence and improve quality of life. Candidates must be licensed in California and experienced in home health or related settings.Key Responsibilities:Conduct comprehensive patient evaluations and develop individualized treatment plansDeliver occupational therapy services in patients' homes in accordance with physician orders and company protocolsInstruct patients and caregivers in use of adaptive equipment and techniques to improve daily living skillsMonitor patient progress and modify treatment plans as neededCollaborate with multidisciplinary team including nurses, physical therapists, and social workersMaintain accurate and timely documentation in compliance with California state and federal regulationsEnsure patient safety and adherence to infection control procedures in home environmentsQualifications:Current and valid Occupational Therapist license in the state of CaliforniaGraduate of an accredited Occupational Therapy programMinimum of 1 year experience in a home health or similar setting preferredValid driver's license and reliable transportationStrong interpersonal, communication, and organizational skillsProficiency with electronic medical records (EMR) systems is a plus Working Conditions:Flexible, per diem schedule based on patient needs and therapist availabilityTravel within a designated geographic region requiredThis position is paid by the visit.
