Licensed Practical Nurse
Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our dynamic healthcare team. The ideal candidate will demonstrate a strong commitment to delivering high-quality, patient-centered care in a variety of clinical settings, including long-term care, assisted living, and acute care environments. Under the supervision of registered nurses and physicians, the LPN will play a key role in supporting patient wellness and treatment outcomes.

Morning: 6:30 AM – 3:00 PM
Evening: 2:30 PM – 11:00 PM
Overnight: 10:30 PM – 7:00 AM
Day: 6:30 AM - 7:00 PM
Overnight: 6:30 pm - 7:00 AM
Key Responsibilities:
Administer medications and treatments as prescribed by licensed healthcare providers.
Perform basic patient assessments, including vital signs and routine physical evaluations.
Assist in the creation and execution of personalized care plans.
Provide support with Activities of Daily Living (ADLs) in long-term care, hospital, and outpatient settings.
Monitor patient conditions and promptly report any changes to the supervising RN or physician.
Document patient information accurately and in compliance with HIPAA and organizational standards.
Collaborate with nurses, physicians, therapists, and social workers to deliver coordinated care.
Perform or assist with IV infusions, catheterizations, tube feedings, and wound care.
Educate patients and families on treatment plans, medications, and health maintenance.
Support discharge planning and participate in case management processes.

Skills & Qualifications:
Active LPN license in good standing.
Experience with EMR/EHR platforms such as (Point Click Care (PCC)) preferred.
Working knowledge of medical terminology, ICD-9/ICD-10, and CPT coding.
BLS certification (Basic Life Support) required.
Excellent communication and time-management skills.
Compassionate and empathetic approach to patient care.
