RN - Hospice & Palliative Care Nurse

Take on the vital role of developing, implementing, and managing individualized patient care plans within a hospice setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and provide compassionate support to patients and their families. This position emphasizes exceptional customer service and respectful communication with all stakeholders.General SummaryResponsible for creating and managing patient-centered care plans for hospice patients ranging from newborns to the elderly. Deliver skilled nursing services aligned with patient and family goals, focusing on symptom management and comfort.Essential FunctionsDevelop collaborative, individualized care plans based on medical and nursing diagnoses.Provide skilled nursing care in accordance with established plans.Promote symptom relief and comfort consistent with patient and family wishes.Communicate assessments and care plans regularly with hospice staff and volunteers.Work closely with physicians, patients, families, and other disciplines to adjust care as needed.Supervise Home Health Aide services following agency policies.Maintain timely and accurate documentation per organizational standards.Foster positive relationships with patients, families, colleagues, and referral sources.Observe and report on patients' physical, mental, emotional, and spiritual conditions.Adhere to HIPAA regulations regarding patient information privacy and security.Minimum Knowledge and SkillsProficiency with MS Office and keyboarding skills.Strong communication abilities.Effective multitasking and prioritization.Application of fundamental nursing principles. Preferred Knowledge and SkillsAdvanced clinical skills relevant to hospice care. Education and ExperienceGraduate of an accredited nursing program.Several years of clinical nursing experience.Preferred: Bachelor of Science in Nursing (BSN).Preferred: Previous hospice and/or home health experience.Preferred: Certification as a Hospice and Palliative Care Nurse. Licenses and CertificationsCurrent Registered Nurse license in the relevant state.BLS certification required within three months of hire.Valid driver's license and proof of auto liability insurance as required.
