Position SummaryThe Staff Occupational Therapist evaluates and treats residents, collaborates with families, physicians, and the interdisciplinary team, and maintains accurate documentation in the medical record. The OT also conducts in-services and training for facility staff on an ongoing basis.Essential Duties and ResponsibilitiesEvaluate residents within 24 hours (or one business day) of physician referral; document clearly if delayed.Develop individualized treatment plans and obtain physician approval.Provide skilled occupational therapy services per physician orders.Supervise Certified Occupational Therapy Assistants (COTAs), therapy aides, and students in accordance with state practice acts.Communicate resident progress, concerns, and care plans with the interdisciplinary team.Participate in Care Conferences, Utilization Review meetings, and Rehabilitation Conferences.Document evaluations, daily treatment notes, progress notes, and recertifications in compliance with state regulations and payer requirements.Instruct families and nursing staff on maintenance programs and caregiver techniques prior to discharge.Participate in discharge planning and recommend appropriate durable medical equipment (DME) to enhance resident independence.Assist nursing department with training of Restorative Aides.Report equipment issues promptly to ensure safe operation.Maintain a positive, professional relationship with residents, families, and staff.Supervisory ResponsibilitiesSupervises Certified Occupational Therapy Assistants (COTAs), therapy aides, and students as applicable. QualificationsEducation and ExperienceBachelor's Degree in Occupational Therapy required; Master's or Doctorate preferred.Experience in skilled nursing or post-acute setting preferred.New graduates welcome to apply. Required Skills and AbilitiesStrong clinical assessment and treatment planning skills.Excellent communication and collaboration abilities.Proficiency in documentation systems such as PointClickCare (PCC) or similar EMR platforms.Ability to read and interpret policies, procedures, and technical guidelines.Strong organizational and time management skills. Certificates, Licenses, RegistrationsCurrent, active, and unencumbered Occupational Therapist license in the State of California.Must maintain continuing education requirements and license in good standing at all times. Physical RequirementsFrequent standing and walking; occasional sitting.Frequent reaching, pushing, pulling, bending, and assisting residents with mobility.Ability to exert up to 100 pounds of force occasionally and 50 pounds frequently when assisting residents.Visual acuity sufficient for documentation and resident assessment. Work EnvironmentThis position operates in a skilled nursing facility environment. Exposure to infectious diseases and healthcare-related hazards may occur.
