Per Diem RN. Monthly Follow-Up Visits Epic Nursing Services, LLC. Massachusetts Pay: $40 to $50 per visit Epic Nursing Services is seeking a compassionate and dependable RN to complete monthly follow-up visits for our AFC and GAFC members throughout Massachusetts. This is a per diem position that offers flexibility and is ideal for a nurse looking to supplement their income while making a meaningful impact in the community. Responsibilities: Conduct monthly follow-up visits with members Monitor and document changes in health condition Follow up with PCPs, managed care plans, and other providers as needed Help coordinate and confirm necessary appointments Maintain clear, accurate, and timely documentation Communicate effectively with agency staff to support continuity and quality of care Qualifications: Graduate of an accredited School of Nursing Active RN or LPN license in Massachusetts Minimum 2 years of experience working with elderly or disabled individuals Ability to travel throughout Massachusetts with reliable transportation Strong organizational, communication, and problem-solving skills Proficient in Microsoft Word, Excel, and Outlook Ability to work collaboratively with staff and leadership Current CPR certification required Must pass a criminal background check This is a great opportunity for a nurse who is organized, independent, and committed to quality care.
