Case Manager/Options Counselor
Performs person-centered comprehensive needs assessments, clinical eligibility counseling, long- term support counseling, options counseling, program pre-screening eligibility, assists with Medicare enrollment at Open Enrollment and client follow-up.
Conducts and/or attends team meetings with appropriate staff and partners as necessary, as well as participates in continuing education and peer counseling workshops and activities. Assists with/provides periodic public education sessions and engages in inter-professional and inter- agency collaboration and education regarding long-term support counseling services.
A bachelor degree in Human Services is required but experience and willingness to continue education with program financial support may be considered to the right candidate.
This is a 30 hour a week position, unless the right candidate has additional experience or education in accounts processing. Duties for this 10 hour a week position include;
perform various bookkeeping, record keeping, clerical functions for processing and reconciliation of ServiceLink/RSVP invoicing and reporting to CAP’s Fiscal Department and BEAS.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
Expected hours: 30 – 40 per week
