Licensure Requirements (Required)Active New York NP or PA License - REQUIRED
Active DEA - REQUIRED
Experience RequirementsExperience in post-acute, acute care, emergency medicine, or clinic settings preferred

Clinical ResponsibilitiesComprehensive patient assessments (H&P)
Develop and adjust individualized care plans
Monitor patient progress and coordinate interdisciplinary care
Collaborate with physicians, nursing staff, and healthcare professionals
Educate patients and families
Maintain compliant and timely documentation
Support & OnboardingStructured 3-month onboarding program
