Registered Nurse – Case Manager (Contract-to-Hire) | Savannah, GALocation: Savannah, GA 31405Shift: Monday – Friday, 8:30 AM – 5:00 PMPay: $50–$53/hr (Contract-to-Hire)Mileage Reimbursement: $0.43/mileAbout the Role:We are seeking a skilled Registered Nurse Case Manager to join our team in Savannah, GA. This role focuses on comprehensive assessment and care coordination for high-risk members across Chatham, Liberty, Bryan, and Effingham counties. You will play a critical role in improving patient outcomes while ensuring cost-effective, individualized care plans.Key Responsibilities:Perform comprehensive assessments of high-risk members.Collaborate with primary care providers to implement individualized care plans with targeted interventions.Continuously reassess and ensure appropriate levels of care.Arrange non-medical support (housing, transportation) to promote treatment compliance.Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.Engage specialty resources to achieve optimal patient outcomes.Maintain accurate and detailed records of clinical, functional, and fiscal outcomes.Identify opportunities for health promotion and illness prevention.Advocate for patients while maintaining privacy and confidentiality.Qualifications:Education: Associate's or Bachelor's Degree in Nursing or related field.License: Current, unrestricted Georgia RN license.Experience/Skills:Proficient with OASIS documentation and accuracy.Knowledge and use of HCHB (Home Care Home Base).Ability to analyze complex medical information and make rational decisions.Case Management certification preferred.Productivity Requirement: 30 units per 40-hour week.Benefits & Perks:Contract-to-hire opportunity with competitive hourly rate.Mileage reimbursement for travel across assigned counties.Work-life balance with M-F schedule.
