Maternal Health Care Coordination Supervisor

Plan A is a nonprofit organization that provides free healthcare services and care coordination in the Mississippi Delta. This role is specifically part of the Healthy Families, Healthy Futures program focused on pregnant women and families with infants up to 18 months.Position SummaryThe Maternal Health Care Coordination Supervisor provides both direct care coordination services to program participants and oversight of a team of Care Coordinators. This vital role involves managing a team of care coordinators, ensuring seamless communication between patients, providers, and community resources, and promoting high-quality, patient-centered, timely support to enrolled families.The Supervisor maintains a reduced caseload while supervising staff, monitoring performance, and ensuring adherence to grant requirements, program protocols, and documentation standards.  The ideal candidate will possess extensive experience in clinical coordination, a strong understanding of the needs of pregnant women and families, and exceptional leadership skills to foster a collaborative and impactful work environment.Key ResponsibilitiesSupervision & Team ManagementSupervise and support a team of approximately 5-6 Care Coordinators, ensuring adherence to clinical protocols and organizational standards.Conduct regular supervision meetings (individual and team-based)Coordinate multidisciplinary care plans for pregnant women, new mothers, and their families, integrating services such as prenatal education, postpartum support, and neonatal care.Support staff with complex participant casesReview and advise staff on outreach and enrollment efforts, in collaboration with the Healthy Start Project ManagerEnsure appropriate resource referrals are made to participantsMonitor staff productivity, caseloads, and service qualityReview documentation for accuracy, timeliness, and complianceCare Coordination Program Management Support data collection, reporting, and quality improvement initiativesEvaluate and provide subject matter input on maternal and child health training curriculaParticipate in audits, site visits, and grant reporting as neededCollaborate with supervisors to maintain strong referral partnershipsQualificationsRequired:Master’s degree in Social Work (MSW) or related fieldMinimum 4-6 years of experience in care coordination, case management, or maternal/child healthExperience working with underserved or high-risk populationsStrong organizational, documentation, and communication skillsResident of the DeltaAble to travel within the communitySupervisory or team leadership experienceAble to lift and move boxes up to 25 lb (specifically for preparation of items to be distributed to parents such as diapers)Preferred:Experience in maternal, perinatal, or infant health programsWork Environment & ScheduleCombination of work-from-home, office, community-based, and/or home visitsLocal travelStandard business hours with occasional flexibility based on participant needsCompensationSalary range: $55,000-65,000Benefits: Paid time off, holidays, 401k match, health, vision, and dental insuranceThis position is funded by a HRSA Healthy Start Grant
