The LCSW will provide individual, group, and family therapy, conduct assessments, and develop treatment plans tailored to client needs. The position requires collaboration with healthcare providers, community resources, and families to ensure comprehensive care. Responsibilities also include case management and documentation to track client progress and maintain compliance with legal and ethical guidelines. Qualifications Proficiency in Social Work and Medical Social Work practicesExperience in Case Management and coordinating care servicesExpertise in Mental Health and Behavioral Health assessment and interventionStrong interpersonal, communication, and problem-solving abilitiesCurrent and valid LCSW licensure in the state of IndianaMaster's degree in Social Work from an accredited programFamiliarity with electronic health record systems is a plusCommitment to ethical practice, cultural competence, and professional development
