Home Health Care Worker

Job SummaryWe are looking for a Home Health Care Worker to join our team! You will be directly working with the assigned individual(s), following a one-on-one plan in the individual(s) home. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets the individual's needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or IHSS experience is a plus. We are a small Agency providing services and support to developmental/intellectually disabled and dual-diagnosed community.ResponsibilitiesAssist individuals with daily activities such as baths and personal hygieneCare for individuals by changing bed linens, doing laundry, cleaning the home, or assisting with personal careMaintain records of individual care, condition, progress, or problems to report and discuss observations with the Agency AdministratorBuild relationships with individuals by providing companionshipEngage individuals in activities of their choiceQualificationsPrevious experience working with Intellectually disabled and dual diagnosed individualsHigh School Diploma or GEDMinimum one-year prior professional experienceDriver's license requiredCurrent full coverage automobile insuranceCurrent CPR/First Aid certification required
