PRN-Registered Nurse - Northern Nevada- Heritage Healthcare Services

OverviewWe are seeking a dedicated and compassionate Registered Nurse (RN) to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks.The RN will manage a caseload of approximately 50-70 clients, ensuring required face-to-face visits every 60 days in the client's home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care.Service Areas & Current Census (subject to change)Cuba, NM - 6 clientsSanta Fe, NM - 33 clientsLas Vegas, NM - 29 clientsKey Responsibilities:Conduct in-home supervisory visits with EPSDT and VA clients every 60 daysEnsure compliance with all federal, state, and organizational regulations and standardsAssess client well-being, safety, and overall care needsDocument visit findings accurately and in a timely mannerCommunicate concerns, observations, and recommendations to appropriate team membersCollaborate with caregivers and interdisciplinary team members to ensure coordinated careMaintain complete, accurate, and up-to-date client recordsProvide education, guidance, and resources to clients and their familiesAdhere to all company policies, procedures, and confidentiality requirementsQualifications:Current, active Registered Nurse (RN) license in New MexicoMinimum of 1 year of nursing experience (home health or community-based care preferred)Strong assessment, documentation, and organizational skillsExcellent communication and interpersonal abilitiesAbility to work independently and manage a flexible scheduleDemonstrated commitment to compassionate, patient-centered careValid driver's license and reliable transportationMust live in New MexicoCompensation & Benefits:Competitive pay and comprehensive benefits packagePaid mileage reimbursementCompensation for travel time and administrative time (including scheduling)Opportunities for professional development and career advancementSupportive, collaborative work environmentWhy Join Us?At Heritage Home Healthcare, we are committed to delivering compassionate, personalized care that helps individuals maintain independence and dignity in the comfort of their own homes.
