MMIT Program Coordinator

Essential functions Train and model Montessori Moments in Time program. Assure understanding of how we bring residents up the Maslows Hierarchy using Validation and Montessori Practices. Implement the 5 directional paths Participate in marketing the memory care neighborhood to referral sources. Model validation/cuing techniques while assisting/caring for residents. Assure and facilitate a seven-day a week activity program based on the activity minimum standards while assuring residents are guided through ADLs programming and meals throughout waking hours. Assist per request with resident screenings. Completion of Life Stories through resident and family interview and memorable experience boards on all residents, create up to date resident interest checklist. Completion with Administrator of Quarterly Progress Report Facilitate Support Group Monthly Participate in MMIT Workshops and MMIT Luncheon Attend Monthly Support Calls Participate in Sherpa duties Assure engagement kits are stocked and available to staff. Training of department heads and MC staff in Montessori Moments in Time program Monthly ongoing MMIT training according to monthly training calendar Support care staff in use of five key directional paths and using past routines when performing residents with care activities (activities of daily living such as bathing, dressing, grooming and toileting). Attend and participate in all MC resident service and care plan meetings. Assist with identifying and transitioning residents from Al to MC. Participate in all required drills and training. Ensure sound infection control practices are in place that promotes a healthy environment. Ensure that resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, privacy and respect. Operate department within established budgetary guidelines. Development of monthly activity calendar and newsletter Able to recognize escalating behaviors and intervene in a safe productive manner using Validation, Montessori practices and identifying through Maslows where resident is at on triangle when behavior occurs. Assure reporting of safety issues to the medical provider and authorized responsible party as instructed by Administrator. Assure all incidents or change of condition are reported to DOW timely. Regularly assists with unit safety checks of the alarm systems and securing of unit doors. Any other duties assigned by the Executive Director. Customer service Responsible for ensuring that all employees are providing excellent customer service to internal and external customers. Perform job duties for residents and with team members in a courteous and professional manner. Take initiative to ensure resident safety and satisfaction is a priority. Meet and greet visitors in a friendly, helpful manner (Visitors include anyone who visits the community; specifically, current residents families and friends, prospective residents and their families, referral sources, vendors and regulators). Answer phones appropriately, according to company and community standards. Communicate professionally and cooperate with supervisor and all community personnel. Residents rights Ensure compliance and understanding of all regulations regarding residents rights. Training Attend training sessions as directed by supervisor. Conduct MMIT orientation and ongoing monthly training Other Follow & communicate Meridian policies and procedures. Support FB postings with LEC and Team The job description provides a framework for the job; other duties may be assigned as necessary. Communicate effectively with Executive Director/management staff/co-workers/residents and family members to perform assigned duties and responsibilities Minimum eligibility requirements 2 - 3 years or more in the care of senior adults and adults with cognitive decline is preferred. Basic knowledge of Alzheimers disease and related dementias and desire to serve residents and families of the memory care neighborhood. Ability to work within an environment that supports the goals set forth in the MMIT Program. Leadership skills and the ability to coordinate care across disciplines (including activities, dining services, etc.) Must be able to communicate effectively with residents, families, staff, community and State officials and general public. Must have compassion for and desire to work with the elderly. Must demonstrate the ability to work responsibly as a team member as well as an individual. Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others. Practice and promote Meridian Policies and Procedures, Mission Statement, Core Values and Founding Principles. Previous supervisory and/or management experience in a related healthcare setting. Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and community. Must have strong documentation skills. Must have experience in training and staff development. Must pass criminal background check. Must meet all health requirements.
