Case Manager

Location: Springfield, OH, 45505Job Description:The Case Manager will be responsible for providing case management services to clients in need. This includes assessing client needs, developing and implementing care plans, coordinating services with other providers, and monitoring client progress. The Case Manager will work closely with clients, families, and other professionals to ensure that clients receive the best possible care.Responsibilities:Conduct initial assessments of client needsDevelop and implement care plansCoordinate services with other providersMonitor client progress and adjust care plans as neededProvide ongoing support and guidance to clients and familiesCollaborate with other professionals to ensure comprehensive careMaintain accurate and up-to-date client recordsParticipate in team meetings and case conferencesStay up-to-date on best practices and trends in case managementRequirements:Bachelor's degree in social work, psychology, or related fieldExperience in case management or related fieldExcellent communication and interpersonal skillsAbility to work independently and as part of a teamStrong organizational and time management skillsKnowledge of community resources and servicesAbility to maintain confidentiality and professionalism
