Residential Service Coordinator
DescriptionCommunity Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.We are actively seeking a Residential Service Coordinator in Las Cruces, NM. The Residential Service Coordinator uses best practices and technology within a community-based setting to assist, train and supervise individuals in activities of daily living and socialization skills. Using discretion and independent judgement, the Residential Service Coordinator coordinates and monitors the assessment and treatment planning process; the delivery and follow-up of patient care; and assures that the appropriate service plans are followed. This individual also serves as an advocate and liaison for and on their behalf.ResponsibilitiesDevelop service plans and ensures individual assessments are completed to determine if goals and objectives are being metGather the necessary information for the applicant to be approved as a waiver participantSecure a residence in the community that meets the individual’s welfare and safety needsCreate and implement the service plan, making revisions every six monthsIdentify, develop, and coordinate community resources and supportMaintain all documentation of service delivery and billingAssure that the assessment of need for a nursing facility level of care will be initiated on the anniversary of the initial determination (PRI/Screen)Initiate a re-evaluation of the level of care when the waiver participant has experienced a significant improvement in his/her ability to function independently in the communityCommunicate and document employee’s progress to the employer and support people, including family, residential staff and other professionalsEmployees must cooperate with the licensee and department staff in any inspection, inquiry or investigationMinimum RequirementsAssociate’s degree and practical experience as a case manager working with people with disabilitiesValid driver’s license with a satisfactory driving recordComplete all state and agency required training per state guidelinesAbility to present facts and recommendations effectively in oral and written formExperience with problem solving against multiple prioritiesAbility to establish a comfortable and supportive relationship with individuals receiving supportsEmployment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testingWorking ConditionsMay be required to travelMay be exposed to various medical conditions and communicable diseaseWhy Community Options?
