MEDSTAR HEALTH is seeking a compassionate and skilled Physical Therapist for Home Care services in Howard County within our Healthcare & Medical Services department. In this role, you will provide one-on-one, evidence-based rehabilitative care to patients in their homes, helping them restore function, improve mobility, and safely remain in their communities. The Physical Therapist Home Care, Howard County position is responsible for performing comprehensive physical therapy evaluations in the home setting, identifying functional limitations, and developing individualized plans of care for a diverse adult and geriatric population. You will deliver hands-on interventions including gait and balance training, therapeutic exercise, manual techniques as appropriate, neuromuscular re-education, and functional mobility training within the patient's natural environment. You will also provide clear instruction to patients and caregivers on home exercise programs, safe transfers, fall prevention strategies, and appropriate use of assistive devices to promote independence and quality of life. Working autonomously in the field while supported by the broader MEDSTAR HEALTH team, you will document all assessments, treatments, and progress notes accurately and promptly in the electronic medical record, ensuring compliance with regulatory and payer requirements. You will collaborate closely with physicians, nurses, occupational therapists, social workers, and case managers to coordinate comprehensive, patient-centered care plans, communicating changes in patient status and contributing to interdisciplinary case conferences. The role requires strong clinical judgment, effective time management, and the ability to prioritize visits across Howard County while maintaining a high standard of service and professionalism.
