Psychotherapist Nursing Homes (Part-Time)
We are seeking a licensed psychotherapist to provide mental health services to residents in skilled nursing and long-term care facilities. This role focuses on delivering evidence-based, client-centered therapy to older adults and individuals with complex medical and psychosocial needs. The ideal candidate will conduct assessments, develop individualized treatment plans, maintain accurate clinical records, and work in compliance with all applicable Illinois and federal regulations governing behavioral health services in nursing facilities.
Key Responsibilities
Conduct comprehensive psychosocial assessments in accordance with Illinois Department of Public Health (IDPH) and facility requirements
Provide individual psychotherapy and, when appropriate, group or family interventions tailored to the nursing home population
Develop and implement evidence-based treatment plans (e.g., CBT, supportive therapy, grief counseling, trauma-informed care) with measurable goals
Monitor resident progress, adjust treatment as needed, and coordinate care with the interdisciplinary team
Maintain timely and compliant clinical documentation, including progress notes, treatment plans, and Medicare/Medicaid billing documentation
Collaborate with nursing staff, medical providers, social services, and external referral sources to support holistic resident care
Provide crisis intervention, suicide risk assessments, and safety planning as required
Adhere to Illinois licensing laws, federal CMS guidelines, HIPAA, and professional codes of ethics
Participate in case consultations, interdisciplinary care plan meetings, and quality improvement initiatives
Support continuity of care, including discharge and transition planning
Required Qualifications
Active Illinois license as one of the following:
Licensed Clinical Social Worker (LCSW) – Master’s degree in social work from a CSWE-accredited program, completed required supervised clinical hours, and passed the ASWB clinical exam
Licensed Clinical Psychologist – Doctoral degree (Ph.D. or Psy.D.) in psychology from an APA-accredited program, completed supervised clinical hours, passed the EPPP and Illinois Laws & Rules exams
Additional requirements:
Experience providing psychotherapy to older adults or in medical/long-term care settings preferred
Proficiency in clinical documentation and use of electronic health records (EHR)
Strong interpersonal, communication, and time management skills
Ability to work independently while maintaining professional and ethical standards
Working Conditions:
Part-Time 20-25 hours a week
Flexibility in setting work schedule, with work hours falling between 7:00 am and 9:00 pm.
Continuously meet minimum required hours of direct services to clients each week, allowing remainder of work hours of documentation, meetings, trainings, and supervision
Will meet with 20 patients or more, may fluctuate
