Job Description SummaryAgency is seeking a VHSP/ Community Service Coordinator responsible for providing case management and intakes in order to assist families participating in our organization's services and/or who are at-risk of homelessness in identifying appropriate housing opportunities. We are looking for a responsible candidate to perform a variety of administrative tasks including coordinating The Improvement Association's VHSP/CSBG program activities in accordance with the funding source(s) guidelines to ensure that all service areas receive community resources to promote the Association's self-sufficiency efforts. The VHSP/Community Services Coordinator will also provide training to customers in regard to different housing opportunities, budgeting, maintaining a household, and leveraging supportive services that will assist the participant household to maintain or obtain permanent housing and other duties that are deemed necessary.DutiesTo familiarize oneself with community resources to maintain an effective service referral system to address customer's needsTo conduct follow-up activities to ensure delivery of needed services and to measure service effectivenessTo submit status/progress reports to funding sources, Executive Director and/or other appropriate administrators, as necessaryIdentify and develop program-specific outcomes to ensure program effectiveness and report outcomes to funders and the community. Design and manage program services directly related to those outcomes.Implement a strong case management component and services focused on moving customers out of homelessness quickly as well as preventing homelessness, making homelessness rare, brief and non- recurring.To conduct follow-up activities to ensure delivery of needed services and to measure service effectivenessDevelop and manage a comprehensive outreach/ working relationship with stakeholders, including other community members, especially landlords. Implement a landlord recruitment and retention plan. Maintain awareness of changes in market and community processes that may impact services; bring forward prospective recommendations to deal with expected changes.Align programs with Best Practices and Housing First Model.RequirementsMinimum of a Bachelor's Degree, preferably in Human Services Field and/or comparable work related experience. Must have some computer, typing, proofreading/editing, oral, written communication, and public relation skills. Must possess reliable transportation and willing to work in other offices when necessary. Must become a certified Housing Counselor within 12 months and/or when the certification becomes available.
