JOB REQUIREMENTS: PURPOSE AND SUMMARY: Ensures residents receive thenecessary care and services, through a multi-disciplinary approach, toattain or maintain their highest practicable physical, mental andpsychological well-being. Provides residents with the nursing care andassessment for establishing interventions within a plan of care and anenvironment which promotes maintenance or enhancement of their qualityof life; all in accordance with Park View Health Center\'s policies andprocedures, Federal and State regulations, the Nurse Practice Act,Standards or Nursing Practice, and the nursing assessment process.ESSENTIAL FUNCTIONS: 1. Delivers skilled nursing care, administersmedications as ordered, transcribes physician orders, and ensures thatdirect care is provided skillfully, safely, completely and accurately. Manages, directs, and provides assignments to the certified nursingassistants by providing guidance-driven leadership in delegating directnursing care tasks as appropriate. This includes assigning neighborhoodtasks, monitoring the productive work flow of the neighborhood to ensureresident care is completed, providing clear communication withappropriate direction to the neighborhood team of CNAs, and providingfollow-up guidance with education as needed. 3. Completes accuratenursing assessments in a timely manner. Identifies resident changes incondition, and directly assesses and observes residents with a change incondition. Documents within the medical record the observation andassessment of the resident change in condition. Follows up withappropriate changes to nursing short term care plan and nursingcomprehensive care plan. Completes follow-up assessments for identifiedresident and neighborhood concerns. ADDITIONAL ESSENTIAL DUTIES: 1.Accurately and timely documents resident assessments, pertinent residentfocused information, weekly resident progress notes,resident/POAHC/Guardian notification, and notification of theresident\'s physician/physician extender within each resident medicalrecord. 2. Critically thinks through resident concerns and formsproblem-solving solutions by identifying interventions on residentshort-term care plans, consulting with interdisciplinary team members,discussing concerns directly with residents and resident family members,and offering assessment information to RN neighborhood supervisors orthe RN shift supervisor for further assessment for care planninginterventions. 3. Maintains effective and professional communicationwith physicians, physician extenders, community healthcare entities(hospitals, clinics etc.), all interdisciplinary departments,supervisor\'s and administration to provide assessment information andcommunicates requests for consultation or guidance for resident focusedconcerns and changes in condition. 4. Understands the nursing departmentchain of command and reports acute incidences or concerns in a timelymanner to RN Shift Supervisor and the RN Neighborhood Supervisor, ifapplicable. 5. Demonstrates ability to organize and prioritize dailyassignments by effectively completing assigned nursing tasks withinscheduled shift to include medication pass, resident assessments,documentation and transcription of physician orders. 6. Accurately andthoroughly completes transcription of newly signed physician orders,physician visits, consultant visits and documents the information withinthe resident\'s medical record. 7. Demonstrates flexibility in workhours and work assignments as assigned by maintaining the ability toadapt to a changing work environment and frequent interruptions. Staffnurses will work every-other-weekend and be assigned to a Holidayrotation schedule. 8. Monitors neighborhood supplies used for residentcare to ensure appropriate quantities are maintained within eachneighborhood, this includes but is not limited to; medications, skin orwound care supplies, tube feeding or oral nutritional supplements, foleycatheters, and adaptive equipment. 9. Completes consents for medicationsand safety equipment by delivering education and obtaining signaturefrom the consenting party. 10. Thoroughly assesses resident vital signs,labs, diagnostic tests, and consultation reports for acute concerns andwill communicate concerns to primary physicians. 11. Participates in theadmission, transfer, and discharge process for residents. 12. Functionsas a resident advocate and responding promptly to requests and concerns. Assists certified nursing assistants with resident direct care asthey are able including but not limited to; transfer assistance,meal-time assistance, and obtaining vital signs. 14. Supports guidanceand education in policy and procedure to prevent and prohibitmistreatment, neglect and abuse of residents or misappropriation ofresident\'s property. 15. Demonstrates a professional demeanor bymaintaining a positive and cooperative attitude towards assigned dutiesand responsibilities; exhibiting poise during stressful situations orincidences; delivering courteous effective communication with residents,visitors, all employees, interdisciplinary team members, supervisors andadministration; and accepting of guidance, education and direction fromnursing management 16.
