Job Description The Physical Therapist PRN at Capstone Healthcare of Daingerfield plays a critical role in delivering high-quality rehabilitative care to patients in a Skilled Nursing Facility (SNF) setting. This position focuses on evaluating, planning, and implementing individualized physical therapy treatment plans to improve patients' mobility, reduce pain, and enhance overall functional independence. The therapist collaborates closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care. Flexibility and adaptability are essential, as this PRN role requires providing therapy services on an as-needed basis to meet fluctuating patient census and acuity. Ultimately, the goal is to support patients in achieving optimal recovery outcomes and improving their quality of life within the SNF environment.As a Physical Therapist, you will:Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.Applies concepts of infection control/universal precautions in performing patient careAssure continuity of quality patient care delivered with appropriate documentationProvide effective communication to patient/family, team members and other health care professionalsPromote agency philosophy and administrative policiesMaintain and complete all documentation within company stated guidelines.RequirementsSuccessful completion of accredited physical therapy program and state licensure exam.Valid Physical Therapy license1 year of home health experience preferredCPR certificationValid Driver's licenseValid Auto InsuranceHomeCare HomeBase experience preferredPhysical DemandsMust be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.Must be able to lift, carry, pull, or push up to 100 lbs..
