LPN Case Manager
We are looking for an immediate hire of an energetic, compassionate case manager to coordinate the care of elderly and disabled individuals in the Home and Community Based Waiver Program. The following requirements are needed:
LPN
Experience in the Home and Community Based Waiver Program preferred (not required)
Must display Effective communication and organizational skills
Multitasker
Must pass all background checks and drug screening
Job Type: Full-time
Pay: $16.00 - $20.00 per hour
