In addition, the Harmony Healthcare Long Island has 4 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. We treat patients regardless of income, residency or immigration status.The Harmony Healthcare Long Island offers a stable employment opportunity with a growing company, and competitive base compensation along with health and dental insurance, paid time off, 401-K with company match, paid holidays, employee discounts and much more.JOB TITLE: Licensed Practical NurseREPORTS TO: Nurse ManagerThe following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.RESPONSIBILITIES:Provides nursing care under the direction of the registered nurse or practitioners, i.e. responsible for the nursing assessment of all patients; assumes an active role in interviewing patients and case managementAssists practitioners with on-and off-site patient carePerforms nursing procedures as needed, i.e. injectionsMaintains quality nursing care, nursing notes and plansPerforms lab work, i.e. blood drawingAssists with patient education and counselingAssists in training of volunteers and residents as neededMust be familiar with emergency procedures and protocolsMust be familiar with clinical policies and programsMust be familiar with referral procedures and back-up sourcesCharts and records the care administration of medication and treatmentsParticipates in on-site and off-site screening projects, special programs and health education workshops as neededActs as the patients' advocate and liaison for preservation of patient's rightsAssists with compilation of health statisticsOther duties as assigned.QUALIFICATIONS:Licensed Practical Nurse in the State of New York;Basic CPR (BCLS) Certification required;At least two years of work experience as a Licensed Practical Nurse preferred.Capacity for covering LPNs at all sites if neededBilingual in English and Spanish/Creole.SALARY: $60k-70kMORE INFORMATION: This is a non-exempt position.Job Type: Full-timePHYSICAL DEMANDS:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
