Care Navigator

HealthMe is transforming how Americans access and pay for specialty healthcare through transparent, bundled pricing. We're cutting out the middlemen and putting providers, patients, and employers back in control.We're looking for a motivated, detail-oriented Care Navigator to join our growing team. In this role, you'll be the frontline connection between HealthMe and the patients we serve — identifying members who need navigation support, managing referrals in our platform, and ensuring every interaction is handled with empathy and professionalism.What you'll doConduct patient outreach to identify members needing navigation support and initiate referrals in HealthMeBuild rapport with patients, explain health benefits, and maintain consistent communication throughout their journeyCollect and document demographic, provider, and clinical information accurately and in compliance with HIPAAManage scheduling, follow-ups, and case tracking; escalate complex issues as neededSupport reporting on metrics, utilization, and outcomes; contribute to workflow improvementsWhat we're looking forHigh school diploma required; associate's or bachelor's degree preferredExperience in healthcare navigation or case managementExcellent communication skills; bilingual is a plusComfortable in a fast-paced, evolving environment; able to work independentlyCommitted to confidentiality and HIPAA compliance
