Lead DSP (Direct Support Professional)

Daily Activities 3-5 yrs. special experience as a Lead DSP. Administers medications to individuals as assigned Observes individuals for any changes in behavior or health that may have resulted from medication administration Ensures that individuals are dressed neatly and appropriately and are well groomed at all times. Assists and encourages individuals to care for immediate personal needs such as:toileting, washing their hands and eating Prepares nutritional meals based on individuals needs and preferences Encourages individuals in the preparation of meals and snacks as appropriate totheir individual plan and behavioral plan Keeps residence- both interior and exterior- and resident personal space: clean,organized and tidy. Encourages them to participate in home beautification and upkeep as appropriateaccording to the individual plans. Teaches individual daily skills such as: household chores, laundry, cleaning, andwashing dishes as appropriate. Documentation and Observation Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status Reports any noted problems to the Program Coordinator and immediately completes Written Incident Report(s) Reports all related medical related incidents to the day Program Coordinator or Delegating Nurse Acts as representative for Axis Care Inc. in a professional manner at all times to families, service coordinators, state officials, community partners, local business Maintains client confidentiality Communicated with Coordinator when items related to individual personal needs and personal space needs.
