Do you believe that housing is healthcare?BHcare is seeking a compassionate, proactive Housing Case Manager to support individuals with a history of opioid use who are experiencing homelessness or are at risk of becoming homeless. This vital role helps clients navigate the housing system, connect with critical resources, and build long-term stability and recovery.This is a full-time position, 37.5 hours a week, Monday-Friday 8:30-4:30 PM the time can have flexibility. Through multidisciplinary collaboration, evidence-based practices, and harm reduction principles, we empower individuals to achieve stability and wellness.Position SummaryThe Housing Case Manager works directly with individuals who have opioid use histories to help them obtain and maintain stable housing. This role blends outreach, advocacy, and intensive case management to reduce barriers to housing and promote long-term stability. The Housing Case Manager collaborates closely with internal clinical teams and community partners to support recovery and prevent homelessness.Key ResponsibilitiesClient Engagement & SupportBuild trusting, supportive relationships with clients through consistent contactAssist individuals in identifying housing options and navigating the rental processSupport clients in developing individualized housing stability and recovery plansConnect clients to community-based services and assist with housing transitionsRespond flexibly to evolving client needs, including crisis situationsCollaboration & CoordinationPartner with internal programs, housing providers, and community agenciesFacilitate access to housing, benefits, employment, and healthcare resourcesParticipate in regular case reviews, team meetings, and cross-program collaborationDocumentation & ReportingMaintain timely, accurate documentation in the electronic health record (EHR)Track progress toward client goals and support program outcome reportingEnsure compliance with all documentation, confidentiality, and data standardsProgram & Community InvolvementRepresent BHcare at community meetings, housing-related events, and with partner agenciesAssist with transportation for clients when neededDeliver services using Housing First and harm reduction principleWe are an Equal Opportunity Employer. QualificationsEducationHigh School Diploma or equivalent requiredBachelor’s degree in Human Services, Social Work, Psychology, or a related field preferredExperience & SkillsExperience working with individuals with mental health and/or substance use disordersExperience working with individuals who are homeless or from marginalized populationsDemonstrated ability to engage effectively with diverse communitiesWorking knowledge of Housing First and harm-reduction-based service deliveryFamiliarity with employment, housing, and human services systems in Greater New Haven strongly preferredTechnical SkillsProficiency in Microsoft 365Ability to navigate and document effectively within an Electronic Health Record (EHR) systemTraining (preferred But Not Required)Harm ReductionMotivational InterviewingDe-escalation techniquesLanguage SkillsBilingual Spanish preferredTransportation RequirementsValid driver’s license requiredMust maintain an acceptable driving recordMust be insurable at a reasonable rate under the organization’s auto insurance plan
