Job Description Locum - Internal Medicine Physician - Eureka, CA Start Date: End Date: Ongoing Reason for Coverage: Vacancy - Long-Term Permanent Search Provider Type: Physician Coverage Type: Scheduled Clinical Hours Only EMR System: Epic Practice Setting: Outpatient Patients Per Shift: 18 Does your Facility require physicians to be board certified?: Yes Fellowship: N/A Minimum Board Certification: Certified What is the license requirement to bid?: Licensed in CA only Estimated Credentialing Timeframe (Days): 44 Candidate CVs need to have month & date on their work experience w/all gaps in employment accounted for. Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD Monday-Friday 8-5 Mix of peds/adults A decision is typically made within 7-14 days APC's with 3 years of experience in similar setting (see other AR) BE will be considered if truly BE (5 years or less since residency) Active DEA needed before start Credentialing takes 30-90 days The following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30 day notice for ending all assignments.
