Patterns Hiring is seeking a travel Outpatient Physical Therapist for a travel job in Clear Lake, California. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 05/04/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Clear Lake, CA | ️ 13 Weeks | ⏱️ 40 hrs/week – 8:00a–4:30p (Mon–Fri) Seeking an experienced Physical Therapist – Home Health with 2+ years experience in home health settings. Must be proficient with OASIS documentation and Homecare Homebase (HCHB). The PT will provide in-home evaluations, treatment plans, and patient-centered care, focusing on improving mobility and functional independence. Must have a valid driver’s license and be comfortable traveling between patient homes. Optional weekend shifts available if desired. Certifications required: Active PT License + AHA BLS/CPR (DPT required if licensed after Jan 2016) Patterns Hiring Job ID # .
