Physical Therapist Location: Ontario, OH Agency: Pride Health Pay: Competitive weekly pay (inquire for details) Shift Information: 5 days x 8 hours Start Date: ASAP About the Position Pride Health is hiring a Travel Physical Therapist, Home Health/Hospice to support our client’s medical facility in Ontario, OH 44906. This is a 3 month contract assignment (convertible to permanent based on your work performance). and a great way to start working with a top-tier healthcare organization! Specific Duties: Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines. Develop and implement individualized treatment plans in collaboration with physicians. Deliver evidence-based therapy to relieve pain, restore function, and improve mobility. Monitor and document patient progress, reporting changes to physicians and nurse supervisors. Educate patients, families, and caregivers on treatment plans, progress, and functional limitations. Requirements: Education: Experience in home health or related clinical settings preferred. License(s): Active Physical Therapist license Certification(s): Current CPR certification.
