Home Health Registered Nurse Case Manager

Job Description Description of the Role: We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a full-time basis. In this role, you will be responsible for telephonic care management and support to patients in Allentown, PA and the surrounding counties. Responsibilities: Reviewing patient assessments and developing individualized care plans with the care team Ensuring all skilled nursing care, including wound care, medication management, and IV therapy is provided as ordered Coordinating and communicating with healthcare team members to ensure comprehensive and effective care is provided Monitoring patient progress and adjusting care plans as needed Educating patients and their families on proper care techniques and self-management Documenting Transfer and Discharge Oasis as needed Requirements: Valid Registered Nurse (RN) license in the state of Pennsylvania Minimum of 1 year of experience in home health or similar setting Minimum of 1 year of experience in a case management role Excellent clinical and critical thinking skills Strong communication and interpersonal skills Ability to work independently and manage time effectively Valid driver's license and reliable transportation Benefits: Competitive compensation Flexible schedule Opportunity to make a meaningful impact on patients' lives Supportive and collaborative work environment About the Company: Interim Home HealthCare is a leading provider of home health services, with a focus on delivering exceptional care and support to individuals in their own homes.
