Perform comprehensive PT evaluations addressing: Strength, balance, endurance, and functional mobility Gait deviations and fall risk Transfers, bed mobility, and stair negotiation Activity tolerance and cardiopulmonary response Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readinessAddress Functional Decline Related To Recent hospitalization Acute illness or injury Deconditioning and mobility lossEducate Patients And Caregivers On Safe mobility techniques and transfer strategies Fall prevention and home safety Use of assistive devices and mobility equipment Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standardsQualifications Required Active Physical Therapist (PT) license in the state of Florida Degree from an accredited Physical Therapy program (DPT preferred) Current CPR certification Valid driver’s license and reliable transportation Strong clinical reasoning, communication, and time-management skillsPreferred / Highly DesiredExperience in one or more of the following: Acute care or hospital-based physical therapy Hospital at Home or transitional care programs Medicare Skilled home health physical therapySkilled In Gait training and balance rehabilitation Fall risk assessment and prevention Functional mobility and discharge planning Post-acute and medically complex patient care Comfortable practicing independently in a home-based acute care setting Familiarity with EMR documentation systemsAbout Activa Home HealthActiva Home Health delivers high-end, patient-centered care through innovative programs designed to meet patients where they are—including hospital-level care at home.
