Case Manager II Family Preservation (Diversion Truancy Program)

We are a non-profit organization which includes Diakon Child, Family, and Community Ministries.General Responsibilities:Responsible for serving as a resource person, case manager, counselor, and trainer to support the youth and families of the Diversion Truancy program.Maintains a caseload of five (5) youth and families.Establishes and maintains professional and supportive relationships with youth, birth families, referring personnel, treatment providers, school districts and others involved in the lives of children and youth.Schedules, completes paperwork necessary (ISP), and participates in initial Treatment Team meetings and quarterly reviews.Education/Training: A BA or BS degree in social service or a related field of study normally requiredExperience: A minimum of one (2) year's experience in a related child welfare or human service position normally required.ADA: ability to bend, stoop, sit, stand, reach and lift items weighing up to 50 lbs.; valid driver's license; visual and auditory skills.Compensation and Benefits:Diakon offers a competitive compensation and benefits package.Day 1 Benefits include medical, dental, vision, and prescription drug coverage.Tuition Assistance - up to $15,000 annuallyPaid Time Off401(k) plan with employer matchVoluntary short-term disabilityLong-term disabilityLife insuranceReferral bonuses of up to $3,000.Staff receive excellent training through a comprehensive paid general orientation including customer service training.
