Social Worker Care Manager Full-time On-site position!Relocation provided!Summary:Seeking a Social Worker - Care Management who will be responsible for leadership, coordination, and operational management of the Care Management Department within a large acute care hospital setting. Work collaboratively with Physicians, nursing leadership, multidisciplinary healthcare teams, patients and families delivering patient-centered care.Responsibilities:Provide leadership and daily oversight of the Care Management Department including staffing, scheduling, workflow management, and departmental performance.Supervise and mentor care coordinators, case managers, social workers, and support staff.Implement best practices and evidence-based care management strategies.Support hospital financial integrity by ensuring appropriate utilization management and discharge planning.Requirements:Master's Degree in Social Work (MSW) from an accredited programWashington State Clinical Independent Social Worker (LICSW) LicenseMinimum 6 years of experience in Case Management within an acute care hospital settingMinimum 2 years of leadership experience at the Manager or Director levelStrong knowledge of hospital care management processes including utilization management, discharge planning, and care coordination.Excellent leadership, communication, and interdisciplinary collaboration skills.Benefits:Comprehensive benefits package including healthcare, retirement plans, and paid time offFor more information, contact:Manisha Palli
