Nurse Practitioner - Pulmonary - SNF / Long-Term Care - Brady, TX
We are seeking a compassionate, experienced APRN to provide patient-centered care in skilled nursing and rehab facilities across Texas, focusing on reducing hospital readmissions and enhancing patient outcomes.Perform assessments, develop treatment plans, diagnose, and manage acute/chronic conditions.Prescribe medications, document all care accurately, and collaborate with interdisciplinary teams.Support preventive care, patient education, and adhere to infection control and regulatory standards.Qualifications: Active TX APRN license, BLS & ACLS, minimum 1 year of nursing experience (preferably in SNF/post-acute care), strong assessment and communication skills.Schedule: Part-time with competitive pay ($120K-$180K/year), ongoing education, and clinical support.Work Location: In person in Brady, TX.Our mission is to streamline patient care through real-time response, quality assurance, and thorough documentation to improve patient outcomes and facility efficiency.
