Telehealth Physician (CCM/RPM) | Remote, Florida/Multi-State📍 Remote (United States)🔹 About the Role We are seeking licensed US-based Physicians to provide high-quality virtual care for patients with chronic conditions, primarily diabetes and cardiovascular diseases. This role focuses on teleconsultations, Chronic Care Management (CCM), and Remote Patient Monitoring (RPM) to improve continuity of care and long-term patient outcomes.🔹 Key Responsibilities • Conduct telehealth consultations (video/audio) for patients with chronic conditions • Manage patients enrolled in CCM programs • Analyze RPM data (e.g., glucose and blood pressure trends) to proactively prevent acute events • Develop and maintain individualized care plans • Perform regular follow-ups and adjust treatment plans as needed • Prescribe medications and ensure adherence to clinical guidelines • Collaborate with care coordinators and support staff • Maintain accurate and timely documentation in the EMR🔹 Qualifications • MD or DO degree from an accredited institution • Active and unrestricted medical license in one or more US states (preferable in Florida/multi-state licensure preferred) • Board-certified or board-eligible in Internal Medicine, Family Medicine, or related specialty • Experience managing chronic conditions, especially diabetes and cardiovascular diseases • CCM and/or RPM experience preferred • Prior telehealth experience is a plus🔹 Skills & Competencies • Strong clinical decision-making skills • Comfortable with telemedicine platforms and digital health tools • Ability to interpret RPM data and act proactively • Excellent communication and patient engagement skills • Strong documentation practices • Comfortable working in virtual/asynchronous care environments • Data-driven approach to preventive care🔹 What We Offer • Fully remote work environment • Opportunity to work in a fast-growing, tech-enabled healthcare model • Support from care coordinators and operational staff • Competitive compensation (depending on licensure and experience)🔹 Impact • Improve long-term outcomes for chronic disease populations through continuous, data-driven care • Help reduce hospitalizations and acute events through proactive monitoring and timely interventions
