Care is in the form of spiritual counseling, prayer, devotional ritual, and grief and loss support. The hospice chaplain is part of an interdisciplinary team that supports people at critical junctures in their lives, always making a difference. Care delivery primarily takes place in a patient’s home or in a community where the patient resides.Hospice Chaplain Responsibilities:Develops and maintains therapeutic relationshipsConducts comprehensive spiritual assessmentProvides therapeutic interventionsProvides effective patient/family teachingSupports the death/dying processCollaborates in providing patient careCreates timely and accurate documentationAssures patient safety. Partners with community chaplains and spiritual counselorsUtilizes resources effectively and efficientlyAdheres to Bloom standards and continuously improves processes/servicesMaintains and enhances professional skillsAdheres to high standards of personal and professional conductHospice Chaplain Minimum Qualifications:Bachelor's degree in Counseling, Divinity, Theology, Ministry or Pastoral Care from an accredited schoolMinimum of two years' experience providing spiritual support and counseling to clients and family members in a health care settingDemonstration of expertise in spiritual assessment and counseling, and in concepts of death and dying issuesKnowledge of concepts and issues for dying for adults and children. The pay range for this role in the state of Colorado typically falls between $65,000-$75,000 annually with the potential for performance-based bonuses and other benefits.
