Social Worker, Palliative Care

Position#Summary The Palliative Care Clinical Social Worker will provide clinical support and address the psychosocial needs of patients and families throughout the hospital and outpatient setting in response to consultation with the Palliative Care Program. The PCCSW will perform psychosocial assessments, provide counseling and support, coordinate care, provide patient/staff and community education, and monitor/report the outcomes of Palliative Care consultations in conjunction with the Palliative Care Program. The PCCSW will plan, develop, implement, coordinate, and evaluate Palliative Care services within the Palliative Care program. The delivery of Palliative Care will be enhanced through activities such as education, program development, quality improvement, consultation care coordination, and discharge management.#This position will be instrumental in transitioning patients and providing education about end-of-life care.# Primary Position Responsibilities 1.Performs and documents psychosocial assessments, care plans, and Palliative Care screenings 2.Plans, coordinates, and implements educational and outreach services for patients, families, hospital staff, and the community 3.Monitors and reports outcomes of Palliative Care referral consultation in conjunction with the Palliative Care Program. 4.Assist the Palliative Care team and/or directly facilitate patient-family meetings for goal setting and care planning. 5.Serves as a mentor and trainer to assist hospital staff with developing primary palliative Care Skills (ie,. case managers, social workers, and nurses 6.Participates in the development and review of appropriate clinical protocols and quality improvement initiatives. # Qualifications Minimum Education: Required: Master#s Degree in Social Work Minimum Years of Experience (Amount, Type and Variation): Required: At least 1 year of experience in Clinical Social Work Experience in Geriatrics, Oncology, or Inpatient hospital medicine. Preferred: Experience in either palliative care or hospice preferred. Certification in palliative care/hospice social work preferred. License, Registry or Certification: Required: NJ Licensed Social Worker Preferred: NJ Licensed Clinical Social Worker; Certification in hospice/palliative care Knowledge, Skills and/or Abilities: Required: Knowledge of death, dying, grief, and loss issues Excellent communication, counseling, and interpersonal skills. Essential are critical thinking, creative problem-solving, and proficient organization, and planning skills. Program development skills to assist with the TJC accreditation process.
