ECM Program Supervisor
About the Role
Drive day-to-day ECM operations and lift team performance so high-needs Medi-Cal members get coordinated, person-centered care. You’ll coach Care Managers/CHWs, keep quality and compliance tight, and remove roadblocks so clients thrive.
What You’ll Do
Lead & Coach: Supervise ECM staff, run huddles, balance caseloads, and develop skills.
Own Quality: Track KPIs (outreach, engagement, care plans), audit charts, and close gaps.
Support Clients: Escalate complex cases, champion client voice, and promote person-centered care.
Partner Well: Interface with leadership, MCPs, and community providers; contribute in case conferences and audits.
What You Bring
Bachelor’s in Social Work, Nursing, Public Health, Human Services, or related (Master’s preferred).
2+ years in a lead/supervisory role in healthcare, behavioral health, or social services.
Hands-on experience with Medi-Cal populations, care coordination/case management.
Working knowledge of ECM, managed care, and California health programs.
Bonus: Bilingual (English/Spanish), local resource savvy, Trauma-Informed Care, Motivational Interviewing.
