Care Coordinator MSW Weekend

Hourly Pay Range:$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position Highlights:Position Care Coordinator MSWLocation: Glenbrook, ILFull Time: 40 hoursHours: Flexible hours: 8-hour shifts Monday-Friday 8:30a-5:00p OR 4-10 hr. shifts 8:00a-6:30p or 8:30a-7:00pEvery 2-3 weekend rotationCoverage for 1 winter and 1 summer holiday require What you will do: Provide comprehensive direct patient continuum of care, case management servicesCoordination and referral to health and social welfare resources and discharge planning in conjunction with the Continuum Care Manager or Care NavigatorFunctions as integral member of a collaborative and interdisciplinary teamSupports Nursing services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients and in maintaining the quality of service deliveryWill also serve as patient advocate What you will need:License: LSW or LCSW license required Education: Master's Degree from an accredited school of social work required Certification: CPR BLS Certification required-American Heart Association (AHA) Experience: Two years' post-graduate experience preferred; healthcare and/or mental health experience preferred Skills: Experience with Electronic Medical Record (EMR) platform preferred.
