What you’ll do in this role: Hospice patient identification, follow-up, and management in the hospital Manage Hospice referrals for assigned area, which may come from several sources including but not limited to: Hospitals, Discharge Planners, Physicians Work closely with the insurance department Review charts to determine eligibility for program(s) Determine Hospice level of care needs for each referral Call/ conduct visits with patients/caregivers to explain and discuss Hospice services Process appropriate documents including physician orders/certification Communicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient We are looking for compassionate Hospice Admissions Registered Nurse with: RN license in the state you work Associate degree or higher from an accredited School of Nursing Two years of RN experience, hospice experience preferred Current CPR and BCLS certification Current driver’s license We are an equal opportunity employer and value diversity at our company.
