Job Title: Case Manager - Housing Department Location: Bend, Oregon Organization: Cascade Youth and Family Center (A Program of J Bar J Youth Services) Employment Type: Full-Time (40 hours/week) Position Summary: Cascade Youth and Family Center (CYFC), a program of J Bar J Youth Services, is seeking a dedicated Case Manager to join our Housing Department. This role is integral in developing positive relationships with individuals experiencing homelessness, understanding their basic needs, and connecting them with vital resources in the community. The Case Manager will assist clients in accessing healthcare, food, employment opportunities, and housing, while also providing ongoing support and guidance toward achieving self-sufficiency. This role requires a compassionate individual who can effectively coordinate resources, manage budgets and documentation, and foster relationships with property management companies to ensure smooth housing transitions. The position involves direct outreach to those experiencing homelessness and includes collaboration with CYFC programs and community resources. Key Responsibilities: Foster a positive team environment that delivers quality service through active listening, problem-solving, and timely responses. Collaborate with clients to develop and implement individualized plans for independence, focusing on self-sustainability through education, skill-building, and employment opportunities. Assist clients in creating and managing realistic budgets for current and future needs. Coordinate and attend meetings and appointments that align with clients’ schedules. Facilitate housing searches, accompany clients through the process, and ensure a clear understanding of lease agreements and housing expectations. Conduct monthly inspections of housing units, documenting conditions and ensuring compliance with housing standards. Meet regularly with clients to assess their progress toward goals and provide ongoing support. Work with Furnished Hope to ensure clients have fully furnished living spaces. Ensure case management activities align with established goals and objectives. Complete specialized training in crisis intervention, human trafficking, suicide prevention, harm-reduction, and trauma-informed care as required by Family Youth Service Bureau (FYSB) and state regulations. Assist the Housing Coordinator in creating life skills groups tailored to the needs of youth. Participate in SPDAT/HEAT intakes and HCV yearly applications to ensure accurate client information. Provide continuous support and advocacy for runaway, homeless, and at-risk youth, including connecting them to necessary services and resources. Maintain accurate documentation and records for program evaluation and reporting purposes, ensuring compliance with grant standards. Collaborate with staff, agencies, and volunteers to coordinate programs that deliver the best possible services for youth. Expand the agency’s network and resource development through community engagement and cooperative activities with other organizations. Provide warm hand-offs and navigate resources to ensure clients receive appropriate referrals. Support the Housing Program Manager with related projects and initiatives. Qualifications: Experience in case management or a related field, preferably within a housing or youth services context. Strong organizational skills and the ability to manage multiple priorities. Excellent communication and interpersonal skills. Knowledge of community resources and the ability to make appropriate referrals. Ability to work flexible hours, including nights, weekends, and holidays. Commitment to supporting vulnerable youth populations and promoting their well-being.
