Position Summary:Responsible for the full client care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients' health and human services needs.Position Description:Under minimal supervision, coordinate and provide care that is safe,timely, effective, efficient,equitable, and client-centeredMaintaincaseload andutilizationbymaintainingregular contact with clients tomonitorand assess progressHandle case assignments, draft service plans, review case progress anddeterminecase closurefor review and/orin consult with supervisorDeliveron-going services to clients which mayincludecrisis intervention, advocacy, medication monitoring, transportationassistance, financial budgeting,housingassistance,and referral and linkage to community resources as neededAssistin helping clients achieve wellness and autonomy through supportive counseling andfacilitatingmultiple care aspects (care coordination, information sharing, etc.)Develop and/or implement individualized client treatment or service plans and ensures compliance with applicable standards and agency policiesDocument all clinical activities to meet specific program,funderand credentialing entity criteriatimelyand with accuracyParticipatein treatment team andassistsin making recommendations for client status changesMayassistin providing education and/or group curriculumAdhere to professional standards as outlined by protocols,rulesand regulationsComplete agency, accrediting body, and funder required trainingOther duties as assignedRequired Knowledge, Skills and Abilities:Ability to learn case management principles, healthcare management and reimbursement requirementsExcellent organizational and time management skillsEffective verbal and written communication skillsAbility to relate to and interact with a wide variety of peopleAdvanced level problem solving skills and ability to multi-task while prioritizingAbility to utilize technology including electronic service documentation systemBasic computer skills with ability to navigate Microsoft applications including but not limited to Word, Outlook, Excel, Teams. and Power PointJob Requirements:Associate's degree in behavioral scienceorrelated field, orequivalent work experienceas allowed/required by regulatory bodiesDriver License and valid auto insuranceEssential Functions/Physical Demands:Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching.
