Summary:Ensure you read the information regarding this opportunity thoroughly before making an application.Under the guidance of the Interim Housing Senior Manager, the case manager provides comprehensive services to homeless individuals and families in the Safe Parking Program. The role focuses on helping participants achieve self-sufficiency and permanent housing through outreach, assessments, service planning, and assistance with entitlement applications. The case manager also facilitates referrals and housing research and addresses stability needs.Essential Functions:• Maintain confidentiality in all matters pertaining to the participants.•Develop effective and trusting professional relationships with the individuals/families to facilitate the provision of services.•Maintain the assigned caseload of participants.•Assist participants in completing their Housing & Services Plan and Budget within three days of enrollment to the program.•Meet with participants twice a month to develop/update the housing service plan (HSP) to identify barriers.•Complete intakes and provide orientation to individuals/families, including the review of program guidelines and consequences for non-compliance, a tour of the facility, and scheduling case managers' appointments.•Adhere to documentation standards set by the program and agency policies, ensuring compliance with HIPAA policies and practices.•Maintain accurate client data in the Homeless Management Information System (HMIS).•Maintain up-to-date and accurate documentation in client files for service coordination/case management, housing placement, rental assistance, and move-in assistance.•Submit case notes into HMIS within 24 hours of engagement using the DAP (Data, Assessment, and Plan) format.•Input participants’ documents into files upon receipt.•Update information, track services (i.e., Housing Stability Plan creation/updates) and referrals, and complete case notes of services provided to the participant within 24 hours following the provision of the services.•Conduct regular (daily/weekly) wellness checks and safety inspections to monitor the upkeep, safety, and security of the facility/premises and the wellness of all.•Complete and submit incident reports and behavioral contracts for program violations.•Provide follow-up of incident reports/behavior contracts to address underlying behavioral issues that prevent the participant from adhering to program guidelines.•Coordinate with individuals/families to implement a life skills curriculum covering areas such as emotional regulation, self-advocacy, parenting, employment, financial literacy, housing search, wellness, and healthy relationships.•Work with the client to complete an individualized housing plan and budget to ensure the household can sustain housing after the subsidy.• Attend weekly case conference meetings.• Collaborate and coordinate with DMH, DPSS, Work Source, and SUD providers.• Provide the Program Manager with all required client information and assessment outcomes.• Prepare monthly reports per program requirements and funder requirements.• Ensure compliance with HIPAA, 42 CFR Part 2, and other funding requirements.• Maintain appropriate boundaries and adhere to the agency’s Code of Ethics and Core Values.• Professionally represent the Agency at meetings, training, and community events.Experience, Skills, Education and Abilities:• Associate’s degree or higher in social services or a related field.• 2+ years of job-related experience working with homeless individuals/families.• Experience in case management and homeless programs is preferred.• Strong organizational skills, attention to detail, and ability to manage multiple tasks.• Familiarity with DCFS and experience placing homeless families into permanent housing is a plus.• Excellent interpersonal and communication skills, with a commitment to maintaining confidentiality.• Proficiency in using computer applications and databases, including HMIS or similar systems.• Knowledge of community resources and services available to individuals experiencing homelessness.• Working knowledge of Microsoft Office and other database programs is essential.• Experience using HMIS and at least one year of case management experience is preferred.• Knowledge of resource development, case management, and documentation is required. xaygatp • Ability to work effectively with clients from diverse cultural, ethnic, and socio-economic backgrounds.Company DescriptionWell-Known Non-Profit Agency serving the homeless communityCompany DescriptionWell-Known Non-Profit Agency serving the homeless community
