We pride ourselves on providing operational guidance to enable nursing facilities to overachieve their quality and operational goals.We invite you to learn more about Social Work opportunities we currently have at the communities we are proud to service.Social Worker:Evaluate patients (existing, new and prospective) based on their needs and limitationsAddress patient concerns and goals while maintaining constant communication with the patient as well as a case managerBuild rapport with patients and their familiesCoordinate care with PT departmentsDevelop care plans for patientsQualifications of Social Worker: Must have an LSW/CSWBachelor's degree required. Master's degree is strongly preferred.Experience as a social worker in a healthcare-related setting.
