Occupational Therapist, OT - Home Health

As a Home Health Occupational Therapist at AccentCare, you provide compassionate, skilled care to patients in their homes. You focus on improving their ability to perform daily tasks, enhancing independence, and supporting aging in place.Key ResponsibilitiesTypical duties include:Assessing patients' functional abilities and developing individualized treatment plans focused on daily living skills (ADLs/IADLs).Implementing rehabilitation techniques, especially for complex neurologic injuries or conditions.Teaching patients and caregivers adaptive strategies, use of assistive devices, and home modifications for safety and independence.Providing skilled occupational therapy interventions in the home environment.Collaborating with interdisciplinary teams (e.g., nurses, physical therapists, physicians).Documenting care, progress, and outcomes per regulatory and company standards.Traveling to patient homes within a designated coverage area.If you bring the following, we'd love to connect:Specialized knowledge of rehabilitation techniques for complex neurologic injuriesHands-on experience in home health, medical-surgical, or rehabilitation center settingsRequired Certifications & LicensuresMaster's degree in Occupational Therapy (or equivalent) with active state licensure or certification in the state of serviceMinimum of two years of experience operating independently with minimal supervisionValid driver's license with the ability to travel across all business locations as needed
