Medical Social Worker

*Overview* A Medical Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing. Services are furnished by a qualified social worker or by a qualified social work assistant under the supervision of a qualified social worker. The position of Licensed Clinical Social Worker shall be a criteria-based position with well-defined responsibility. *Responsibilities* -· Understands and adheres to established Agency policies/procedures. · Provides social work services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing. · Understands and promotes principles of continuous performance improvement. · Participates in the coordination of Agency's services with the services of other community resources; uses community resources. · Observes confidentiality and safeguards all patient information. · Serves as a resource person to employees, patients, physicians and other allied healthcare providers. · Accepts responsibility for regular attendance and punctuality. · Maintains current knowledge of Medicare Conditions of Participation for social work services. · Immediately reports any accident, incident, lost articles, or unusual occurrence to Director of Clinical Services/Nursing Supervisor. · Participates in inservices, case conference and/or continuing education programs. · Maintains contact with community support groups and provides professional expertise as required. · Plans/coordinates all social services within the Agency. · Documents all patient/family services provided as required by Agency policy. · Assists physician and other team members in understanding the significant social and emotional factors related to health problems. · Prepares clinical and progress notes. · Works with the family. · Participates in discharge planning. · Routinely interviews family, extended family and significant others to ensure effective psycho social assessment and placement in alternate care environments. · Ensures compliance with adopted institutional Risk Management, Infection Control and Safety Policy and Procedures. · Maintains strict patient, client confidentiality of all disclosed information. · Maintains patient, client follow-up on all referrals to alternate care environments. · Generates physical assessment on all patients, clients, going to alternate care environments, LE, SNF, ICF, Hospice, Rehabilitation. · Other duties assigned by Director of Nursing. *Experience/Education* - One to two years’ experience in a health care setting and working knowledge of Discharge Planning process. - Familiarity with assisted living facilities and hospital protocols B.S. in Social Work with a master's degree from a school of social work accredited by the Council on Social Work Education. · Licensed Medical Social Worker (PTA) in the State of Texas. · Must be self-directed, motivated, goal oriented. Supervised by: Director of Nursing. · Strong experience in applying Social Service modalities in the patient care environment. With the ability to work with individuals to enlist cooperation of many people to perform/ achieve a common goal. · Working knowledge of computer application. Working knowledge of Quality Assurance process. · Acceptance of philosophy and goals of Agency. · Maintains strong working rapport with outside community, referral agencies, professional associations and community hospitals.
